🛑একটা গুরুত্বপূর্ণ ক্লিনিকাল ইনফো:
🔹KDIGO 2022 অনুযায়ী :

“It is reasonable to withhold SGLT2 inhibitors during prolonged fasting, surgery, or critical illness (because of increased risk of ketosis)”
Key concept হচ্ছে : SGLT2 inhibitor কী করে?

SGLT2 inhibitor (যেমন dapagliflozin, empagliflozin)
➡️ kidney দিয়ে glucose বের করে দেয় (glycosuria)
➡️ insulin level কমে
➡️ glucagon ↑ হয়
➡️ body fat breakdown করে → ketone production ↑

🛑কেন problem হয় এই specific situation-গুলোতে?

1. Prolonged fasting (রোজা/না খেয়ে থাকা)

খাবার নেই → glucose কম
insulin ↓↓ কম

তখন ,body fat ভাঙে → ketone ↑ বাড়ে।

SGLT2i থাকলে → এই ketosis আরও বেড়ে যায়

Risk: euglycemic DKA (dangerous, sugar normal থাকতে পারে)

http://২.surgery/ = stress → cortisol, catecholamine ↑
-insulin resistance বাড়ে ↑
-patient fasting এ থাকে
🫵already ketosis tendency
🫵SGLT2i add হলে → DKA risk ↑↑

৩.3. Critical illness (sepsis, MI, ICU patient)

severe stress → insulin এর কাজ কমে যায়।

catabolic state → fat breakdown ↑ বাড়ে
সাথে,dehydration থাকে অনেক সময়
➡️ SGLT2i → আরও ketone ↑ বাড়িয়ে দিতে পারে।
➡️ so,euglycemic DKA risk high

🛑SGLT2 inhibitor এ DKA হতে পারে even with normal glucose
➡️ একে বলে euglycemic DKA
➡️ diagnose করতে miss হয়ে যায় easily

🔥এই তিন situation-এ SGLT2i temporarily stop করা উচিত:
-prolonged fasting (e.g. Ramadan, NPO)
-surgery (usually 3–4 days before)
-critical illness

শুধুমাত্র আমরা antiHBc positive পাই..
কারণ কী?

কারণ,HBsag এবং HbsAb অলমোস্ট সমানে সমানথাকে।
যার ফলে নিউট্রিলাইজড হয়ে যায়।

qSOFA এর পরিবর্তে বর্তমানে রিকমন্ডেড NEWS2 (National Early Warning Score 2) স্কোরিং করতে পারলে ভালো। এটা করার জন্য ২-৩টা instruments থাকলেই চলবে,
1. Pulse oximeter
এটা দিয়ে Heart rate & SpO2 দেখা যাবে
2. Thermometer
Temperature দেখতে হবে
3. BP machine

🛑elevated blood urea with normal creatinine concentration indicate করে severe bleeding..

🛑কারো যদি upper gi bleeding হয় এবং সে aspirin নিয়ে থাকে,এসপিরিন কান্টিনিউ করতে পারবে।তবে clopidogrel ,warfarin,DOAC মাস্ট স্টপ করতে হবে।

#acute_pancreatitis এর পেইন 🛑

আমরা জানি left upper abdominal pain হয়,ব্যাকে রেডিয়েট করে।
যেহেতু retroperitoneal ,এজন্য এখানে rebound tenderness পজিটিভ থাকবেনা।

ধরুন একটা পেশেন্ট এরকম পেইনের হিস্টরি দিলো?
আপনি ডায়াগনোসিস এস্টাবলিশমেন্টের জন্য serum amylase ,serum lipase, UsG scanning করতে পারেন।
১.serum amylase ডায়াগনোসিসের জন্য খুব স্পেসিফিক না।কারণ intestinal ischaemia,pud perforation ,ruptured ovarian cyst এও এটা বাড়তে পারে।
আবার:
পেইন শুরু হবার ২৪-৪৮ ঘন্টার মধ্যে এটা নরমাল হয়ে যায়,কীডনী দিয়ে excretion হয়।
তবে,
if serum amylase persistently elevated থাকে,এবং পেইনের হিস্টরি থাকে ,এটা indicate করে pancreatic pseudocyst formation...

২.serum lipase more specific...
Serum lipase ≥ 3 times the upper limit of normal (ULN)

এটাকেই clinically significant elevation ধরা হয় for acute pancreatitis

৩.ডায়াগনোসিস কনফার্ম করার জন্য USG করা যেতে পারে।তবে সেটা অপারেটর ডিপেনডেন্ট হওয়ায় আমরা contrast enhanced CT করে থাকি।
এটা অবশ্যই ৩ দিন পর করতে হবে।

First 24–48 hours CT করলে problem কী?

Early phase এ pancreas-এ changes mainly:
-edema
-inflammation
কিন্তু necrosis তখনও fully develop হয়নি
তাই CT misleading হতে পারে
severity underestimate হওয়ার risk থাকে
