Health AI is being built on a shaky scientific foundation nobody wants to talk about.

Not bad algorithms. Not biased datasets. Something more fundamental.

We have stopped looking inside the body after death.

Autopsy rates have been plummeting globally and the pandemic augmented the problem. In sudden unexplained community deaths, formal histological examination is now the rare exception.

This means the post-pandemic cause-of-death data that AI systems are trained on is increasingly derived from clinical inference rather than pathological confirmation.

If novel pathological patterns like cardiac micro-scarring, spike-mediated vascular injury, post-viral immune dysregulation, are contributing to deaths in ways that don't map to existing diagnostic categories, they are invisible to every model trained on coded clinical data.

The AI doesn't know it's learning from incomplete information.

It can't know. That's precisely the danger.

Any governance framework for health AI that doesn't treat autopsy rates as a data quality infrastructure problem is regulating the visible surface of a much deeper failure.

At what point does deploying clinical AI on unvalidated ground truth become a known and accepted patient safety risk?
