BASIC EPIDEMIOLOGY TERMS

• Epidemiology: Study of distribution and determinants of disease in population
• Incidence: Number of new cases in a given period
• Prevalence: Total number of existing cases at a given point of time
• Morbidity: Sickness rate in a population
• Mortality: Death rate in a population
• Endemic: Disease constantly present in a region
• Epidemic: Sudden increase in cases above expected level
• Pandemic: Epidemic spreading across multiple countries

HERD IMMUNITY

• Definition: Indirect protection when sufficient population is immune
• Also called: Community immunity or Population immunity
• Achieved by: Natural infection or vaccination
• Herd immunity threshold for Measles: 95%
• Herd immunity threshold for Polio: 80-85%
• Higher the infectivity of disease, higher the herd immunity needed

DISEASE PREVENTION LEVELS

• Primordial Prevention: Prevent risk factors from emerging
• Primary Prevention: Prevent disease before it occurs (vaccination)
• Secondary Prevention: Early detection and treatment (screening)
• Tertiary Prevention: Reduce disability and rehabilitation

SCREENING TESTS

• Sensitivity: Ability to correctly identify diseased people (true positive)
• Specificity: Ability to correctly identify non diseased people (true negative)
• PPV: Positive Predictive Value
• NPV: Negative Predictive Value
• Ideal screening test: High sensitivity and high specificity

OUTBREAK INVESTIGATION STEPS

• Confirm diagnosis
• Confirm existence of outbreak
• Define cases
• Find cases and collect data
• Determine source and mode of transmission
• Implement control measures
• Communicate findings

IMPORTANT RATES

• Infant Mortality Rate: Deaths under 1 year per 1000 live births
• Maternal Mortality Rate: Maternal deaths per 100000 live births
• Case Fatality Rate: Deaths among diagnosed cases
• Attack Rate: Used during epidemic or outbreak

EXAM TIP
Incidence measures new cases while prevalence measures existing cases. Sensitivity is the ability to detect true positives and is used for screening tests. IMR is the best indicator of health status of a community. These are very frequently asked in NORCET and RRB exams.

🔸 K – Kussmaul breathing
🔸 E – Elevated blood glucose
🔸 T – Thirst (polydipsia)
🔸 O – Osmotic diuresis (polyuria)
🔸 N – Nausea/vomiting
🔸 E – Emesis with fruity breath (acetone smell)
🔸 S – Stupor/confusion (if untreated)
