🧪 HIGH-YIELD TOPIC: LIVER CIRRHOSIS & PORTAL HYPERTENSION 🧪

Highly important for NORCET 2026 & AIIMS. Focus on the life-threatening complications like Esophageal Varices and Hepatic Encephalopathy.

📌 WHAT IS LIVER CIRRHOSIS?
A chronic, progressive disease where healthy liver tissue is replaced by scar tissue (fibrosis), leading to liver failure.
👉 Main Causes: Alcoholism, Hepatitis B & C, and Nonalcoholic Fatty Liver Disease (NAFLD).

📌 COMPLICATIONS (EXAM FAVORITES)

1️⃣ PORTAL HYPERTENSION:
Increased pressure in the portal vein system.
👉 Leads to Ascites (fluid in the abdomen) and Esophageal Varices.

2️⃣ ESOPHAGEAL VARICES:
Dilated, fragile veins in the esophagus.
📍 Emergency: If they rupture, the patient can bleed to death.
📍 Nursing Action: Avoid coughing, straining, or heavy lifting.

3️⃣ HEPATIC ENCEPHALOPATHY:
Caused by High Ammonia Levels because the liver cannot detoxify it.
👉 Key Sign: Asterixis (Flapping tremors of the hand).
👉 Other Signs: Confusion, fustor hepaticus (musty breath).

4️⃣ ASCITES:
Fluid accumulation in the peritoneal cavity due to low Albumin.
📍 Assessment: Shifting dullness and fluid wave test.

📌 NURSING PRIORITIES & MANAGEMENT

1️⃣ Paracentesis (for Ascites):

* Ask the patient to void (empty bladder) before the procedure to avoid bladder puncture.
* Position: High-Fowler’s or Sitting at the edge of the bed.

2️⃣ Hepatic Encephalopathy:

* 💉 Drug of Choice: Lactulose.
* Mechanism: Promotes ammonia excretion via stool (Goal: 2-3 soft stools per day).
* Diet: Low to Moderate Protein (to reduce ammonia production).

3️⃣ Bleeding Varices:

* 💉 Medication: Vasopressin or Octreotide (to reduce portal pressure).
* Procedure: Sengstaken-Blakemore tube (Balloon tamponade) used to stop bleeding.
* 📍 Safety: Keep scissors at bedside to cut the tube if airway obstruction occurs.

📌 CLINICAL LABS
🔴 ALT & AST: Elevated (Liver enzymes).
🔴 Ammonia: Elevated.
🔴 Bilirubin: Elevated (Causes Jaundice).
🔴 Albumin: Decreased (Causes Edema/Ascites).
🔴 PT/INR: Prolonged (Risk of bleeding).

📌 EXAM QUICK POINTS
✔️ Early Sign of Cirrhosis → Fatigue and Weight loss.
✔️ Asterixis (Flapping Tremor) → Sign of Hepatic Encephalopathy.
✔️ Lactulose Goal → 2 to 3 soft stools per day.
✔️ Paracentesis Rule → Empty bladder first!
✔️ Sengstaken-Blakemore Tube → Keep scissors at the bedside.
✔️ Diet for Cirrhosis → High Calorie, Low Sodium (if Ascites), Low Protein (if Encephalopathy).
