The CT depicts a left-sided mediastinal tumour diagnosed as small cell lung cancer (SCLC).
SCLC may cause a paraneoblastic syndrome of inappropriate antidiuretic hormone secretion
(SIADH). A low sodium and a low serum osmolality in combination with a high urine sodium
(>40 mEq·L−1) and a high urine osmolality (usually >300 mOsm·kg−1) are characteristic for a
SIADH.

a. Loop diuretic

A loop diuretic is unlikely to induce hyponatraemia. Almost all cases of severe diuretic-induced
hyponatraemia are due to a thiazide-type diuretic.

b. Diabetes mellitus

Severe hyperglycaemia may cause hyponatraemia but in
this patient, the glucose is only mildly
elevated.

c. Carcinoid tumour

Carcinoid tumours are well-differentiated neuroendocrine tumours that may arise in the lungs
or the gastrointestinal tract. They may cause carcinoid syndrome characterised by diarrhoea and
flushing, which is not the case in the vignette.

d. Squamous cell lung cancer

This type of lung cancer may cause paraneoplastic hypercalcaemia, not SIADH.

A 65-year-old man with a smoking history of 80 pack-years cigarettes is admitted for severe
headache, nausea and muscular weakness. He has a history of diabetes mellitus type 2 and
coronary heart disease. He is treated with insulin, metformin, vidagliptin, torasemide, aspirin
and pravastatin. Serum sodium is 119 mmol·L−1, potassium 4.8 mmol·L−1, glucose 8.7 mmol·L−1
and serum osmolality is 256 mOsm·kg−1 (normal range 280–300 mOsm·kg−1). Urine sodium is
141 mmol·L−1 and urinary osmolality is 584 mOsm·kg−1 (normal range 50–1200 mOsm·kg−1). His
renal function is normal. His thoracic CT is shown below.

The patient suffered from poliomyelitis at the age of 14 years. When getting older, her proximal
muscles became weaker and her kyphoscoliosis progressed, as shown by the chest radiographs.
Because of weakness of the respiratory muscles and kyphoscoliosis, she now suffers from a
decompensated chronic hypercapnic respiratory failure that is partially compensated by a high
HCO3
−. Her pulmonary function is severely restrictive because of neuromuscular disease and
kyphoscoliosis

b. Opiate toxicity

Opiate toxicity may cause acute but not chronic hypercapnic respiratory failure

c. COPD exacerbation

Her pulmonary function is not obstructive.

d. Obesity hypoventilation syndrome

Severe kyphoscoliosis is not a feature of obesity hypoventilation syndrome.

e. Cardiogenic pulmonary oedema

Pulmonary oedema may cause acute but not chronic hypercapnic respiratory failure in older
patients. The chest radiography does not show pulmonary oedema.
