Crew transfer is treated as routine, yet it remains the most exposed operation we conduct at sea. The industry has invested significant attention in pilot boarding arrangements, and that focus has improved compliance, visibility, and enforcement around the ladder itself. What follows immediately after, the transfer of crew, continues to operate with far less scrutiny despite carrying the same, if not higher, level of exposure.

Across ports and anchorages, the pattern does not change. The ladder is rigged correctly, procedures are followed, and the system appears compliant, but the point of failure remains fixed at the interface where a controlled ship meets a moving launch. At that moment, the operation shifts away from systems and into individual execution, where timing, footing, and judgment determine the outcome on an unstable platform without fall protection.
Regulatory frameworks exist and are well defined, yet they do not function as a single, continuous system across the transfer.

The highest exposure in maritime operations occurs in the final few meters before a seafarer steps onboard.

Standing watch has long been treated as a sign of discipline onboard. If you are on your feet, you look alert. That assumption has carried across fleets for years.
But the question is whether it actually improves performance.
There is no STCW requirement to remain standing, and no IMO guidance that links posture to vigilance. Yet in practice, sitting is still discouraged on many bridges.
In reality, watchkeeping depends on sustained engagement—continuous scanning, instrument cross-checking, and staying mentally connected to the traffic situation over time.
A watchkeeper can remain standing for an entire watch and still disengage. At the same time, a seated officer can remain fully involved and aware.
What prolonged standing does introduce is fatigue.
Over long hours, that fatigue begins to affect endurance, and eventually performance.
Modern bridge design already supports seated operation. The capability exists. It is simply not used.
