Diabetic kidney disease is a leading cause of kidney failure and premature death in people with diabetes.1 Obesity is central to this burden, yet risk assessment still relies largely on BMI, which cannot distinguish fat from lean mass or metabolically harmful visceral depots from less harmful subcutaneous depots. In The Lancet Diabetes & Endocrinology, Juan Shi and colleagues1 move beyond the question of how much fat to where it is stored and—crucially—to who carries it.

[Review] Secondary causes of diabetes: a crossroad of endocrinology and oncology

Cancer and diabetes share a bidirectional relationship, with cancer increasingly recognised as an associated factor of diabetes, both from the disease itself and its treatments. In this Review, we aim to: (1) summarise the distinct mechanisms of dysglycaemia arising from cancer and neuroendocrine tumour treatments, including targeted therapies such as PI3K–AKT–mTOR inhibitors, antibody–drug conjugates, immune checkpoint inhibitors, corticosteroids, and somatostatin receptor ligands; (2) examine the often overlooked aspect of the secondary cause of diabetes as an early manifestation of cancer; and (3) highlight research gaps and encourage a collaborative care approach to manage the rising rate of dysglycaemia as a result of cancer and its evolving treatments.

The age of large administrative datasets has arrived with both promise and peril, particularly in perinatal epidemiology. Electronic health records capturing millions of pregnancies and birth outcomes are now widely available, providing unprecedented statistical power and external validity to test a range of hypotheses, including some related to the long-term health effects of prenatal exposure to adverse metabolic conditions.1 Yet, statistical power is a paradoxical advantage.2 These vast datasets can just as easily amplify biased conclusions as they can advance our understanding of maternal and child health.

Implementing this combined genetic–clinical stratification could provide more accurate risk assessment, indicate those at higher risk of recurrence, and ultimately personalise long-term follow-up in patients with Cushing's disease.

The study by Qilin Zhang and colleagues published in The Lancet Diabetes & Endocrinology evaluates the risk of recurrence following successful surgery for Cushing's disease and examines its association with USP8 genotype and tumour size.1 With 435 patients included, it represents one of the largest investigations to date exploring genotype–phenotype associations in Cushing's disease. The findings suggest a robust and clinically relevant predictor of long-term outcomes, which is particularly important given that disease recurrence following initially successful transsphenoidal surgery remains a substantial clinical challenge.
