Ten days after a normal vaginal delivery…

Salma, 31 years old, walked into clinic holding her newborn.
She looked pale.

She said:

“Doctor… I have a terrible headache.
The worst headache of my life.”

• Blood pressure: Normal
• Temperature: Normal
• Neurological exam: Normal

Quick impression:
• Postpartum migraine
• Tension headache

She was given analgesics… and discharged.

Three days later…

She came back to the emergency department.

“The headache wakes me from sleep…
and I’m vomiting.”

Differentials considered:
• Post-dural puncture headache? (No epidural given)

✔️ Therapeutic anticoagulation (LMWH)

Even if there is small hemorrhagic transformation.

Because this is a venous thrombosis —
and the treatment is anticoagulation.

طالبة جامعية، بدون أي تاريخ مرضي يُذكر.

أمها بتقول وهي منهارة:

“بقالها أسبوعين مش طبيعية…
بتصرخ، بتضحك لوحدها،
وتقول إن في حد بيراقبها.”

ليلى كانت:
• متوترة
