زارني شاب يبلغ من العمر 27 عاماً ، يعاني من تقرحات جلدية مؤلمة في المنطقة التناسلية تتكرر سنوياً ، زار في غربته أثناء حدوثها أطباء جلدية ، والغريب أنه يعطى علاجا بدون البحث عن بقية أعراض المتلازمة !!!
عند سؤالي هل تظهر تلك التقرحات في الفم ؟ كان الجواب : نعم ( أكثر من ثلاث مرات ) ..
وعند سؤالي عن أي مشاكل في العين فكان الجواب أنه زار دكتور عيون وقال له التهاب وخذ هذه القطرات !!!

كل طبيب سواء جلدية أو عيون لازم يأخذ القصة المرضية بشكل متكامل، ولازم يحدد نوعية ومواصفات تلك الآفة، ولا يعتمد فقط على شكوى المريض ، لكي نصل للتشخيص الصحيح والسليم بكل بساطة ، وممكن ننقذ روحا بشرية كما في هذا المريض الذي تبين أنه يعاني من مرض بهجة وهي متلازمة تصيب عدة أجهزة في الجسم ، وتكمن خطورة هذا المرض في الآتي :
التهاب شبكية العين وممكن العمى ، حدوث جلطات ، تكون أمهات دم شريانية ( Aneurysms) خصوصاً في الشريان الرئوي وتكون قاتلة ، وكذلك العديد من المضاعفات الأخرى ..
Behçet’s Disease :
Chronic, relapsing, multisystem inflammatory disorder characterized by recurrent oral ulcers + any of the following: genital ulcers, eye inflammation, skin lesions, or pathergy reaction.
It is a type of variable-vessel vasculitis (can affect vessels of any size, artery or vein).
Most Life-Threatening Complications:
🔥 Pulmonary artery aneurysm (massive hemoptysis)
🔥 Severe ocular involvement → blindness
🔥 Neuro-Behçet with brainstem disease
🔥 Large-vessel arterial aneurysms .

Erythema Elevatum Diutinum (EED)
A rare, chronic form of leukocytoclastic vasculitis presenting as persistent red-violet to brown papules, nodules, or plaques, typically over extensor surfaces.

Etiology & Associations:
EED is associated with several conditions, especially:
Chronic infections
• HIV, streptococcal infections, hepatitis B/C
Autoimmune diseases
• Rheumatoid arthritis, celiac disease, SLE.
Hematologic disorders
• IgA monoclonal gammopathy (most classic)
• Multiple myeloma, lymphoma
Sometimes idiopathic.

Clinical Features:
Symmetric violaceous to reddish-brown papules, nodules, or plaques
Located mostly on extensor surfaces:
• Dorsum of hands
• Elbows
• Knees
• Achilles tendon area
Lesions may be fibrotic and firm in chronic disease.
Burning or tenderness may occur.
Chronic → color becomes yellowish-brown due to fibrosis & lipid depositions.

Bullous Type of Erythema Multiform :
A variant of erythema multiforme in which the typical target lesions develop central blistering (bullae).
It can occur in EM minor or EM major, but is more prominent in EM major due to greater epidermal necrosis.
Clinical Features:
1. Skin Lesions
Typical target lesions with:
Central necrosis → forms a vesicle or bulla
Surrounding dusky ring
Outer erythematous halo
Bullae may be:
Serous or hemorrhagic
Rupturing → erosions and crusts
Distribution:
Symmetric on extremities, especially palms, soles, extensor surfaces
May involve trunk in severe cases.
2. Mucosal Involvement
Bullous lesions on oral, ocular, or genital mucosa indicate EM major.
Oral lesions form:
Painful erosions
Hemorrhagic crusting on lips
Etiology:
Same triggers as EM in general:
Herpes simplex virus (most common)
Mycoplasma pneumoniae
Drugs (less common than in SJS/TEN):
NSAIDs
Sulphonamides
Antiepileptics (phenytoin, carbamazepine)
Antibiotics (penicillins)

Erythema Multiforme Major
A severe form of erythema multiforme characterized by:
Typical “target” lesions
Or ± Papular atypical targets
Lesions may coalesce → polycyclic plaques , PLUS;
Involvement of ≥2 mucous membranes (commonly oral, ocular, genital) .
Prevalence: ~0.5–1 per million/year.
